Kneed was developed by Dr Nathan Johns

The Kneed program was developed by Dr Nathan Johns whilst he was a PhD candidate at Monash University with his thesis titled Redesigning rehabilitation to treat persistent pain post-surgery. The aims included developing the best evidenced-based digital pain rehabilitation program that could be made available all over the world.

Dr Johns is a registered specialist medical practitioner in Australia in Rehabilitation Medicine and he is a Fellow of the Australasian Faculty of Rehabilitation Medicine, which is a part of the Royal Australasian College of Physicians.

With his supervisors, 4 articles researching pain after knee replacement were published in peer review journals. The most important of these was a pilot randomised controlled trial of an online self-management programme for people with persistent pain post-knee replacement (Kneed) outlined below.

What the Kneed research study showed

The Kneed trial published here ran over 8 weeks and randomised 60 participants to either 8 weeks of the online Kneed program or to continue with 8 weeks of their “Usual Care” (mostly GP and surgical medical care, medications and physiotherapy).

  • 20/31 of the Kneed group started and completed the Kneed program (10 didn’t start or complete the first module)
  • 28/29 of the Usual Care group completed the trial

At 8 weeks, we found that :

  • 53% of Kneed participants strongly agreed and 21% agreed that they were satisfied with Kneed

  • 58% strongly agreed and 16% agreed that Kneed had been useful for their health and well-being

  • 56% strongly agreed and 17% agreed that Kneed helped them to manage their pain and function effectively

Average pain scores compared between the start of the program and 8 weeks:

  • 5 (17.9%) of the Usual Care group reduced their pain by 50% 

  • 10 (50%) of the Kneed group reduced their pain by 50% 

We asked participants to rate their persistent knee pain since the start of the study:

  • In the Usual care group, 4 (14.3%) rated their pain as much or very much improved 

  • In the Kneed group, 10 (50%) rated their pain as much or very much improved

Why do I still have pain after a knee replacement?

Kneed will help you to understand why pain persists after a knee replacement. Although pain is complex and unique to every person, research has consistently shown multiple reasons why pain persists.

Before knee replacement, pain has usually persisted for a long time due to arthritis and a number of risk factors that heighten the pain experience and lead to changes in the central nervous system and brain.

Surgery creates an inflammatory reaction to help heal but this can create new or worsen existing changes in an already sensitive and highly protective system. Individual risk factors like complications after surgery also make pain more likely to persist.

Is it possible to have no pain after a knee replacement?

Yes! Our research indicates that only 14% of people are pain free at 3 months but that 50% are pain free at 12 months. 

Is Kneed a real program?

Yes! This is a legitimate Australian business enterprise bringing rehabilitation directly to you from the research and clinical knowledge of Dr Nathan Johns, a specialist rehabilitation medicine physician. It is rare to have access to an online pain rehabilitation program as there are very few electronic health programs available to the public outside of research trials.

Is Kneed evidence-based?

Yes! Kneed was developed in a translational research Doctor of Philosophy (PhD) program at Monash University in Melbourne Australia by Dr Nathan Johns. This included a pilot randomised controlled trial of 60 participants, conducted online in Australia between October 2024 and March 2025. 

Will I be guaranteed some pain reduction?

It is impossible to guarantee how you will and engage, learn and respond with Kneed and therefore what your results will be. In the Kneed trial, 50% of participants who completed the Kneed program reported clinically important reductions in pain at 8 weeks.

Is Kneed free?

There is a free introductory trial of Kneed for you to see if it is a good fit for you. There are two paid versions on a weekly or 3 monthly plan.

We expect that if Kneed reduces your pain and improves your self-management that this should reduce your healthcare costs for medication and healthcare visits.

Do I need to pay tax?

You only need to pay tax (GST) if you are an Australian citizen or make the purchase whilst living in Australia. At checkout, Australians will have the 10% goods and services tax added to their price.

Do I have to do the Kneed modules in a specific order?

No – after you complete the Kneed Pain Knowledge 1 module, you have access to the modules in any order, whenever you like, within your paid plan timeframe. If you have completed the Free Kneed Module, this will also unlock the other modules once you have purchased Kneed.

Why do I have to complete Pain Knowledge 1 to unlock the other modules?

We need to know that you are onboard with an understanding of pain neuroscience first as this is one of the keys to success. 

Why haven't the modules unlocked?

You need to make sure that you have clicked the mark complete box at the bottom or top of each page in the Pain Knowledge 1 module or Free Kneed. You also have to pass the quiz in one of these modules!

Can I still see my physiotherapist (or other therapist)?

Yes, Kneed teaches you how to manage yourself but also discusses how this can work in with your physiotherapist (physical therapist). 

Do I need permission from my Orthopaedic surgeon or Family doctor (General Practitioner)?

No, however, you need to be confident that:

i).  There is nothing wrong with your knee prosthesis (e.g. no infection, fracture or loosening)

ii). You are able to safely exercise (how to safely exercise is discussed in Kneed in the exercise module)

Can I see Dr Johns for a consultation?

Yes, but only if you are living in Australia. This can be done in person in Brighton, Melbourne or via videoconference from other states. Appointments require a referral from your GP or surgeon.

What are the system requirements?

You need a reliable email account, a computer, tablet or phone and an internet connection. Kneed was designed for a computer or tablet but can be accessed on your phone.

Do we collect health information?

We do not collect any of your health information in Kneed and you will not be asked for it.
